Intake Form Overview for Supervised Visitation Services
3 Steps to Your First Supervised Visit
- Complete the intake form
- Submit required documents
- Attend a separate interview & orientation via Zoom or in person.
Note: Any case determined by the court as a “flight risk” will not be accepted by SVO.
Our intake form serves as a comprehensive guide. We are here to assist you during this trying time by helping us understand the unique dynamics of your family. It allows our team to structure your visitations in compliance with court orders while maintaining a secure environment that prioritizes your child’s well-being.
Why the Intake Form Matters
We understand that paperwork can be tedious, but the value of this step cannot be overstated. The intake form collects detailed information about both parents, the child(ren), and any relevant legal or medical circumstances. This ensures that visitation monitors are fully informed and equipped to manage visits according to legal guidelines and specific family needs.
Required Information
To move forward with visitation services, both parents must complete and submit the intake form. While some questions may feel personal, having accurate and thorough information is critical in the case of emergencies.
The intake form will request the following:
-
Personal Information: Full names, dates of birth, contact details, email addresses, and physical addresses.
-
Custodial Status: Indicate whether you are the custodial or non-custodial (visiting) parent.
-
Legal Documentation: Copies of any relevant legal documents, such as Divorce Decrees, Custody Orders, Supervised Visitation Orders, Protective Orders, etc.
-
Child Information: Names, ages, known medical conditions, allergies, and emergency contact information.
-
Visitation Preferences: Desired visitation schedules or location requests.
-
Authorized Individuals: Identify any third parties who may transport or participate in visitation, if not already specified in the court order.
What Happens Next?
Once payment has been received, the intake form will be distributed to the email address provided. However, both parties must fill out the intake form. This step must be completed before we can proceed with case approval and scheduling.
By completing the intake form promptly and thoroughly, you're helping to ensure that every visit is handled professionally and respectfully, with the child’s best interest at the heart of every decision.
If you’re searching for supervised visitation centers near me, we are here to provide safe, neutral, and supportive services tailored to your family's needs.
What if the Custodial parent is uncooperative and won't fill out the form?
Ideally, parents work together, but that’s not always possible. SVO does not enforce compliance—we contact the other parent once but do not chase payments or visitation arrangements. Any noncompliance should be addressed with your attorney and the Court.
